I paid via Dynadot for a domain transfer but I had forgotten to unlock the domain from Godaddy. I just unlock the domain on godaddy and I am trying to initiate the transfer again by entering the Authorization code. However Dynadot is asking me to Pay again for the transfer. What should I do ?

We can certainly take a look for you. What's the order ID number you're asking about? Looks you submitted 3 transfer orders today and they've all completed.

looks like the server names didn't transfer...
Sometimes they do and sometimes they don't, any reason why?

If you had name servers set up with your previous Registrar, those should be carried over. If you had something else set up, such as DNS or forwarding, those settings wouldn't be carried over. You actually have to have name servers set for the domain.
